Register as a New Patient
We are welcoming new patients and are keen for you to join the surgery.
You can register quickly and easily using our online New Patient Registration forms. Alternatively, if you prefer, download, print and complete the relevant paper form(s) and hand to a member of our friendly reception team.
Acceptance on to the list is as at the discretion of the practice and we may ask you for proof of identity if required to help us find your medical records or transfer them from your current GP.
We will register you as quickly as we can but please allow up to 3 working days for the process to be completed. Please note that sometimes your medical records may take several weeks to arrive with us.

If you are unsure whether your address is within our boundary please check by entering your postcode in the Check Postcode box above our boundary map. We are only able to register patients who live within our boundary and if you are unsure please ask reception to give you advice on whether you can register with us.
If you move outside of the practice area you will need to re-register with your new local surgery. One of the reasons for this is so you can access the correct range of other community services.

Summary Care Record Consent (SCR)
As part of registering with a GP Practice, patients also need to complete a SCR Patient Consent Preference Form. To maintain continuity of clinical care, we upload certain medical information so that it is available to other healthcare organisations (eg Emergency Departments). Your patient record is held securely and confidentially on the electronic system at your GP practice. If you require treatment in another NHS healthcare setting such as an Emergency Department or Minor Injury Unit, those treating you would be better able to give you appropriate care if some of the information from the GP practice were available to them. This information is shared electronically via the Summary Care Record, used nationally across England.

Text Messaging Service
We utilise SMS text messaging to communicate with our patients. We will not send any clinical or medical information by SMS text message and this service will only be used for:
- Appointment confirmations
- Appointment reminders
- Invitations for long term condition reviews
- Invitations for eligible clinics e.g. Winter Flu vaccinations
- Informing patients about health and wellbeing services.
It is practice policy that all new patients are automatically opted in for receiving text messages from the practice. If you wish to opt-out of this service please contact us.
Temporary Patient Registrations
If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
